Technical Assistance:
1. The Violation Notice portion of inspection reports are required to be posted and readily viewable by the public. They should not be covered by other papers, or in a plastic sleeve behind other pages of the report.

Conducted an unannounced monitoring inspection 11:40am-12:30pm. This inspection was focused on the violations cited at the 2/10/16 inspection. Observed 8 direct-care staff + 51 children. Ratios and supervision were in compliance in all groups. The Director + Assistant Director were not present during the inspection. Children were having indoor free-play, washing hands, and eating lunch during the inspection. The infants were being held, fed, and having play time on the floor. Lunch served: cheese pizza, corn, applesauce, and milk. Record keeping and physical plant violations reduced. Questions about this inspection may be directed to:

Standard #: 22VAC40-185-140-A
Description: Based on records reviewed, it was determined that 1 of 5 children's records reviewed did not include a physical exam. Evidence: Child #7 - The child has been enrolled since 9/18/13 without a physical exam.

Plan of Correction: The physical for the child has been obtained and placed in the child's file. All child files will be audited monthly to ensure all medical forms are present. CD and management team will ensure all enrollment paperwork is submitted before a child's start date.

Standard #: 22VAC40-185-50-A
Description: Based on observations made, it was determined that information about staff and children was not maintained in a confidential manner. Evidence: The Supplemental Information page from the 2/9/16 inspection was posted for public viewing in the lobby of the center. Identifying information about children/staff is found on the Supplemental Information page. The document states, "information found on the Supplemental Information page is confidential and this document is not to be posted in the facility."

Plan of Correction: The posting of violations will be posted leaving out the supplemental page. CD will ensure all future licensing reports will be correctly posted.

Standard #: 22VAC40-185-270-A
Description: Based on observations made, it was determined that areas of the center were not maintained in a clean and safe condition. Evidence: 1. 4's Classroom - The carpet was buckled and not installed tightly to the floor. The many buckled areas of carpet posed as a potential trip hazard. Additionally, the carpet was stained and not clean. 2. School-age Classroom - The carpet was buckled and not installed tightly to the floor. The many buckled areas of carpet posed as a potential trip hazard.

Plan of Correction: work orders were placed with facilities CD will ensure all work orders for facilities issues are placed in a timely fashion and Work Order Numbers will be kept on file to ensure follow through is evident [sic]
