Violations:
Standard #: 22VAC40-185-70-A
Description: Based on documentation reviewed, the Center failed to maintain a complete record for each staff member. Evidence: The LI reviewed 1 staff record, the following information was not available for review: one reference, written information to demonstrate that the individual possesses the experience required by the job position.

Plan of Correction: Information will be completed in Staff record.

Standard #: 22VAC40-185-200-A
Description: Based on staff interviews, the Center could not ensure a qualified program director is regularly on site at least 50 percent of the center's hours of operation. Evidence: 1. A qualified PD was not present during the inspection. 2. Staff reported that a schedule of the Program Director's hours was not available for review. Staff was under the impression that the PD could work at the Center at times the center was closed, i.e. week-ends.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 22VAC40-185-260-B
Description: Based on reviewed documentation, the Center failed to obtain annual approval from the health department for meeting requirements for water supply and sewage disposal system. Evidence: The LI reviewed documentation of the health inspection report, it was dated 03/13/2015. The Center is not currently providing meals to the children. Children bring packed lunches.

Plan of Correction: The Health Inspector will be contacted to complete the sanitation/environmental form for the Center.

Standard #: 22VAC40-185-270-A
Description: Based on observations by the LI, the Center failed to maintain outdoor play equipment in a clean condition. Evidence: The LI observed several pieces of outdoor play equipment in the back play area. The equipment had dust and dirt on the surfaces from outdoor winter storage. One piece of play equipment was stacked inside of another piece of equipment which prevented use by the children.

Plan of Correction: Staff will clean the outdoor play equipment so the children have access for play.
