Comments:
The licensing inspector conducted an unannounced renewal inspection Wednesday, June 15, 2016 from 11:50am to 2:02pm. The children were observed eating lunch and preparing for rest time. Lunch consisted of chicken salad, peas and corn, pineapples, crackers, and milk. The infants were observed being held and playing with their teacher. Interviews were held with staff throughout the inspection. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Three children?s records and two employee records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 5/27/2016 Last shelter-in-place drill: 2/24/2016 Last fire inspection: 2/3/2016 Last health inspection: 5/4/2016 Total enrollment today: 55 Today, the following child to staff ratios were observed: Infants ?4:1 Toddlers ?5:1 Twos & Threes ? 24:2 (rest time ratio) Pre-K ?14:2 School Age ?8:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-240-D-4
Description: Based on a review of medication on 06/15/2016, the center did not ensure that a child with an emergency medication was in the care of a staff member trained to administer that emergency medication. EVIDENCE: Child #1 has an emergency medication that is administered rectally. The center did not have any staff on site trained to administer rectal medication. Any child for whom emergency medications (such as, but not limited to albuterol, glucagon, and epipen) have been prescribed shall always be in the care of a staff member or independent contractor who meets the medication administration requirements.

Plan of Correction: Will have two staff complete certification by Friday, June 17th.

Standard #: 22VAC40-185-280-B
Description: Based on observation on 06/15/2016, the center did not ensure that all hazardous materials were in a locked place. EVIDENCE: 1. In the Pre-K classroom there was a box of cleaning supplies on top of a cabinet. Their were no children in the classroom at the time and the supplies were out of the reach of children. 2. Cleaning supplies were observed in an unlocked cabinet in the kitchen. 3. In an unlocked bathroom off of the kitchen disinfectant spray and other cleaning supplies were observed in an unlocked cabinet and on a window sill in the bathroom. The children do not use this bathroom. Hazardous substances such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children.

Plan of Correction: 1. Corrected on site. Items placed in a locked cabinet 2. Corrected on site. The door is normally kept locked. Door to kitchen locked. 3. Will install a lock on the cabinet today-6/15/2016.

Standard #: 22VAC40-185-520-C
Description: Based on observation on 06/15/2016 the center did not ensure that all diaper ointment or cream was labeled with the child's name.

EVIDENCE:

1. A container of Desitin belonging to Child #3 was observed in Child #3's cubby. The Desitin was not labeled with the child's name.
2. A container of A&D ointment belonging to Child #2 was observed in Child #2's cubby. The A&D ointment was not labeled with the child's name.
If diaper ointment or cream is used, these products shall be in the original container and labeled with the child's name
