- Technical Assistance:
Consultation was given in the following areas: 1. A MAT trained staff member should be on-site during all hours of operation. The RN for the school clinic leaves at 2:00 and would not be available for the last hour of the program. Registration for MAT classes can be found on the VA DSS web site under Professional Training. 2. Adding tummy time to the infant daily communication sheet for easy reference for staff. 3. Moving the large blue mat to fit under the indoor slide to provide additional cushioning at the end of the slide. 4. Verified LA Baby cribs are in compliance with current CPSC federal guidelines by checking on-line product information.
An announced initial visit was conducted from 9:00-12:00 at the facility. No children were in care and will not be on-site until summer school starts July11, 2016. Program has four groups serving infant-three years and supporting teens that are still in school. Mothers and babies are encouraged to spend time together throughout the school day. The school nurse does a daily health check each morning with the parent and child. Policies and procedures, staff records for lead staff, emergency preparation (FA/CPR certification, maps, supplies, and drills) were reviewed as well as menus/nutrition for children. Program participates in the USDA food program so all food is maintained at the center with the exception of special formula. Children and parents are transported by APS school transportation. Model DSS forms will be completed by the parent for child enrollment, injury forms, authorization for medication, sunscreen and insect repellent. Areas of non-compliance are identified in this report. Please sign and date the report and return it to me within 10 calendar days from date of receipt. Standard #: 22VAC40-185-270-A Description: Based on direct observation, two small areas of the space used by the facility are not in clean and safe condition. Evidence: 1. A baseboard mat is ripped in one small place by the exit door causing a potential pinching hazard. 2. The paint is popping off the white "round" wall and in the 2/3's classroom a small area of yellow paint is starting to peel along one wall. Plan of Correction: The school administration will put in a work order to the maintenance staff and areas will be repaired prior to summer school starting.
Standard #: 22VAC40-185-550-B Description: Based on review, the written emergency plan did not contain emergency officer information. Evidence: The emergency plan did not list both an emergency officer and backup officer by name with 24 hour contact telephone numbers. Plan of Correction: Plan was updated during the inspection with names and phone numbers added to plan.
Standard #: 22VAC40-185-550-C Description: Based on direct observation, emergency evacuation maps were missing a secondary route. Evidence: The evacuation maps only had a primary route listed on the map and did not contain a secondary means of exiting the building. Plan of Correction: Maps were updated with a secondary route during the inspection.
