Comments:
An unannounced monitoring inspection was conducted from 10:30am-12:00pm. Upon arrival there were 10 children in care with 4 staff present. A sample of 1 staff and 4 children's records were reviewed. In addition 1 medication was reviewed. Children were observed participating in a science experiment and were making play-doh.

Violations:
Standard #: 22VAC40-191-60-B
Description: Based on a record review the center failed to ensure all employees have completed a sworn statement of affirmation prior to employment. Evidence: Staff #1, hired on 05/16/16, did not have documentation of a sworn statement of affirmation. Staff #1 was working with children at the time of inspection.

Plan of Correction: This was an oversight. Staff #1 signed a sworn statement during the inspection and all other background checks have come back with a satisfactory result.
