Comments:
A complaint investigation was conducted today regarding allegations of staff to child ratios, physical plant and nutrition. Interviews were conducted, classrooms observed and documentation reviewed. There was not a preponderance of evidence to support the allegations regarding physical plant and nutrition, however, the complaint was deemed valid with regard to staff to child ratios. The complaint related violation and other non-related violations are identified in the Violation Notice. Complaint related: No
Description: Based on record review, the center did not obtain documentation of a negative tuberculosis screening no later than 21 days after employment for a staff member. Evidence: Documentation of a negative TB screening was not observed for Staff A, who was originally hired August 18, 2014 and transferred to current center location on June 13, 2016.

Plan of Correction: Documentation has been obtained for Staff A.

Description: Based on record review, staff records were incomplete. Evidence: 1. Documentation of the written information to demonstrate that the individual possesses the orientation training required by the job position was not observed for Staff C. 2. Documentation of at least two references was not observed for Staff B, Staff C and Staff D during the inspection. Documentation of two references for Staff B and one reference for Staff C and Staff D were emailed to the Licensing Inspector after the inspection but the date that the references were conducted was not observed.

Plan of Correction: References have since been obtained for Staff C and D. All future references will be dated.

Description: Based on direct observation, equipment of the center was not maintained in a clean, safe and operable condition. Evidence: The children's couch and chair were observed ripped and torn in the Beginner's (2yrs) classroom.

Description: Based on direct observation, hazardous substances were not kept in a locked place. Evidence: 1. Lysol wipes were observed in an unlocked cabinet above the sink areas in the Pre-K 2 classroom. 2. A spray bottle containing a liquid solution was observed in an unlocked bathroom cabinet in Pre-K 2 classroom. 3. A can of WD-40 was observed in an unlocked bathroom cabinet in the Pre-K 1 classroom. 4. A spray bottle labeled "bleach/water", Lysol wipes, Goo Gone and a can of disinfectant spray were observed in an unlocked cabinet above the sink area in the Pre-K 1 classroom. 5. A bottle of hand sanitizer was observed on the window ledge near the entrance door of the Pre-K 1 classroom. 6. A can of disinfectant spray was observed in an unlocked bathroom cabinet in the Toddler classroom. 7. A bleach/water solution was observed on the changing table in the Beginners (2yrs) bathroom. 8. A can of Lysol spray was observed in the bathroom cabinet and in the cabinet above the sink area in the Intermediates (3yrs) classroom. 9. Three spray bottles with a liquid solution and one spray bottle labeled "bleach/water" were observed in an unlocked bathroom cabinet in the Intermediates (3yrs) classroom.

Plan of Correction: All have been immediately corrected. New cabinet locks were purchased and replaced. Classrooms will conduct licensing checklists.

Description: Based on documentation review, for children from three years of age to five years of age, there was not not one staff member for every 10 children. Evidence: 1. According to a "Staff to Child Ratio Sheet" on June 13, 2016 at approximately 10:00am there were 21 children supervised by 2 teachers in the AM Preschool classroom, which exceeded the 1:10 teacher/child ratio. 2. According to a "Staff to Child Ratio Sheet" on June 13, 2016 at approximately 10:00am there were 13 children supervised by 1 teacher in the Pre-K 1 classroom, which exceeded the 1:10 teacher/child ratio. 3. According to a "Staff to Child Ratio Sheet" on June 13, 2016 at approximately 15:15pm there were 15 children supervised by 1 teacher in the Intermediates (3yrs) classroom, which exceeded the 1:10 teacher/child ratio.

Plan of Correction: Staff has been re-trained and all staff has signed off on Licensing Student/Teacher Ratios. Student counts will be done more frequently by front desk staff. New, more effective ratio count sheets are being used.

Description: Based on record review, the center did not obtain a completed sworn statement or affirmation prior to employment for staff. Evidence: A completed sworn statement or affirmation was not observed for Staff C and Staff E.

Plan of Correction: Statements have since been obtained for Staff C and Staff E.
