Comments:
An unannounced renewal inspection was initiated today at 10:50am. The physical plant, 4 staff files, 4 children's files, annual fire and health inspection, staff training, children's activities, medications, emergency procedures, drills and supplies were inspected today. There were 34 children under the supervision of 6 staff. During the inspection, children were observed enjoying free play, learning about geography, coloring, preparing for and eating lunch. The children bring their lunches from home each day they are in care. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 3 staff certified in MAT (medication administration training). Should you have any questions, please contact me at mary.wizbicki@dss.virginia.gov.

Standard #: 22VAC40-185-130-B
Description: Based on file review, 1 out of 4 children's files does not have documentation of additional immunizations once every six months for children under the age of two years. Evidence 1. Child #1's (25 months old) most recent updated immunization on file was completed when he was 21 months old.

Plan of Correction: The parents will be asked to bring the most recent immunization in to keep on file.

Standard #: 22VAC40-185-60-A
Description: Based on file review, 1 out of 4 children's files does not have documentation of the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. Evidence 1. Child #2 has two emergency contacts listed, but one does not have their address.

Plan of Correction: We will ask the parents for this information.

Standard #: 22VAC40-185-320-B
Description: Based on observations, not all bathrooms are equipped with all required supplies. Evidence 1. The bathroom that children in classroom #3 use does not have paper towels.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-570-E
Description: Based on observations, not all of the infant bottles were labeled with the child's name and date. Evidence 1. There was one bottle in the Infant classroom refrigerator that did not have the date labeled on it.

Plan of Correction: Parents normally label the bottles each day. We will remind them and make sure this is completed daily.
