- Technical Assistance:
The licensing inspector conducted an announced inspection in response to a self-reported incident regarding an error in feeding infants. The self-report was received by the licensing department on 06/13/2016. An inspection was initiated on 06/16/2016 and completed on 06/23/2016. The licensing inspector was on site from approximately 10:30am until 12:10pm on 06/16/2016. Interviews with staff were conducted, observations were made in the classroom and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the self-report is determined to be valid. See the violation notice for more details. Standard #: 22VAC40-185-40-E Description: Based on interviews and review of center policies, the licensee did not ensure they complied with their own policies and procedures required by the Child Day Center Standards regarding bottle feeding Infants. Evidence: The facility's Infant feeding policies and procedures state: "Bottle Feeding and Infant Mealtime: Before feeding the child, match the label on the bottle to the child's face. Confer with your co-teacher that you have the correct bottle and the correct child. " Staff #1 stated on 06/10/16 she fed a bottle to Infant A that belonged to Infant B. Staff #1 stated she did not check the labeling on the bottle that identified the infant. Plan of Correction: Teachers were re-trained on safe and proper bottle feeding procedures. They were taught the "talk out loud" method as well. Teachers and management will hold each other accountable.
Standard #: 22VAC40-185-340-A Description: Based on interviews, the center did not to provide adequate care and protection for an infant in care that received another child's breast milk. Evidence: Based on interviews, the following was determined: 1. Staff stated on 06/10/16 at approximately 12:20pm, Staff #2 said she was warming a bottle for Infant A in the bottle warmer. After a few minutes, Staff #2 stated she removed the bottle for Infant A and placed the bottle in a container of cool water because the bottle was too warm. 2. Staff #3 placed a bottle for Infant B in the bottle warmer and after a few minutes, removed it and placed it on the counter and prepared to feed Infant B. 3. After hearing Staff #2 state she had heated the bottle for Infant A, Staff #1 stated she grabbed a bottle that was sitting on the counter and began to feed Infant A without checking the attached label that contained the child?s name and date. 4. Staff #3 stated she went to grab the bottle for Infant B and could not find it. Staff #3 stated she saw the bottle for Infant B being fed to Infant A and informed the staff and a member of management. 5. Staff #1 stated Infant A drank approximately one to one and a half ounces of breast milk from a bottle belonging to Infant B. Plan of Correction: Teachers were re-trained on "Bottle Feeding and Infant Mealtime" policies. They use the match the name on the label to the child's face. Teachers and management will hold each other accountable.
