Discussed with the assistant director of the importance of keeping doors which lead to electrical rooms, boiler rooms, and/or HVAC rooms locked and inaccessible to children. During the inspection we also discussed some of the rugs in the classrooms specifically the Preschool Room was beginning to show some wear. Discussed options of cleaning the rugs or purchasing new ones.
An unannounced monitoring inspection was conducted July 8, 2016 from 11:30am-1:45pm. There were 51 children (infants-5yrs.) directly supervised by 11 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, singing songs at circle time, preparing and eating lunch. There was a sufficient supply of materials for the children which were organized. Standard #: 22VAC40-185-270-A Description: Based on observation, the center did not keep areas and equipment inside the center maintained in a clean and safe condition. Evidence: 1) In Infant Room 2, there is peeling paint on the left back wall of the classroom. 2) In the Preschool Room bathroom, a sheet of paper was used to cover a section of wall that needed to be patched and painted located to the bottom right of the toilet. 3) In Infant Room 2, the play kitchen sink was found to be taped and covered due to the plastic sink being broken and having jagged edges. Plan of Correction: - Paint was removed and repainted. - Area by toilet has been patched and repainted. - Tape was removed following licensing visit and staff spoken with.
Standard #: 22VAC40-185-520-C Description: Based on observation, the center did not have a diaper ointment labeled with the child's name. Evidence: In infant room 2, a diaper cream was not labeled the child's name. Plan of Correction: Staff will ensure diaper cream is labeled using tape and permanent marker.
Standard #: 22VAC40-185-550-B Description: Based on record review, the emergency plan did not specify assembly points for evacuation and shelter in place drills. Evidence: Upon review of the emergency plan, plan indicated when a need for evacuation or shelter in place occurs to meet at designated areas but no areas were identified specifically. Upon interview with assistant director, the assistant director named the specific areas staff were told to meet but confirmed no documentation in the emergency preparedness plan. Plan of Correction: Management will review and revise emergency plan to include designated areas for emergency evacuation and shelter in place.
Standard #: 22VAC40-185-560-G Description: Based on observation, the center did not have a food container clearly dated and labeled in a way that identifies the owner. Evidence: In Infant Room 2, a sippy cup containing milk was not labeled with a date or the child's name. Plan of Correction: Staff will be retrained on proper labeling procedures and parents communicate with about labeling container properly at drop off.
