Comments:
An unannounced monitoring inspection was conducted from 9:20 am-11:50. At the time of entrance, 87 children were in care with 20 staff members present. Children were observed during snack time, engaged in group activities, diapering, hand washing, painting and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The violation was reviewed with the center director. Standard #: 22VAC40-185-270-A
Description: Based on observation areas and equipment of the center are not maintained in a safe and operable condition. Evidence: The red and blue Step 2 wagon on the middle playground was observed with a cracked wheel. The inner door handle in the Apricots classroom was broken with the metal plate detached from the handle.

Plan of Correction: The wagon was immediately removed from the playground. The wagon will be trashed if a replacement wheel can not be purchased. The door handle will be fixed my maintenance tomorrow.
