Comments:
The licensing inspector conducted an unannounced renewal inspection July 27, 2016 from approximately 10:30am to 1:20pm. The following ratios were observed with a total of 52 children present in the Preschool building during the inspection: Infants - 3 children with 1 staff Toddlers - 5 children with 1 staff Twos - 5 children with 1 staff Threes - 10 children with 1 staff Fours - 15 children with 2 staff School Age - 14 children with 1 staff The children were observed engaged in a variety of activities during the licensing inspection. Infants were observed eating lunch. The Toddlers were observed singing songs and dancing, playing with baby dolls, riding indoor cars, and preparing for lunch. The Twos classroom was observed listening to music, looking at books and eating lunch. The Threes were observed playing with play-doh and cookie cutters. The Fours classroom was observed reviewing sight words and creating sentences using sight words. The school age children were observed playing math Bingo, looking at books, playing games and eating lunch. Lunch consisted of meatball pasta with mixed vegetables, pineapples and milk. All areas of the center including classrooms, hallways, kitchens, playgrounds, and bathrooms were inspected. The center was equipped with toys and supplies and items were available to the children. The following information was reviewed: updated injury prevention plan, injury reports, daily sheets, daily attendance, practiced emergency drill logs, parent agreements, and staff training information. The center's first aid kit and emergency supplies were inspected. The director stated medication is not being administered at this time. The center provides transportation for field trips. Transportation documentation was reviewed during the inspection. Last documented health inspection: 05/11/16 Last documented fire inspection: 07/11/16 During the inspection, five children records and four staff records were reviewed. The risk profile tool will be used for the licensing recommendation once all of the necessary paperwork is received by the licensing department. The renewal application was received prior to the expiration of the current license. Until the new license is received, the provider is legally operating. Standard #: 22VAC40-185-160-A
Description: Based on review of four staff records, one record did not contain documentation of a negative tuberculosis screening within the required time frame. Evidence: During the inspection, the licensing inspector observed the record for Staff #3 (hire date 06/27/16) did not contain documentation of a negative tuberculosis screening . Staff are required to submit results of a negative tuberculosis screening that has been obtained within the last year or within 21 days of beginning employment.

Plan of Correction: Per director - The staff member has obtained the T.B. test and needs to submit the document.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #3 (enrollment date 05/23/16) did not contain the physical address for one of the two emergency contacts listed.

Plan of Correction: Per director - We will get the required information from the parent today at pick-up.

Standard #: 22VAC40-185-500-B
Description: Based on observation of classrooms, the facility did not ensure the surface for diapering changing area was nonabsorbent. Evidence: During the inspection, the licensing inspector observed the diaper changing pad located on the diaper changing table in the Infant and Toddler room contained multiple small tears creating a nonabsorbent surface.

Plan of Correction: Per director - We will buy new pads today and replace the torn ones.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on review of background checks for board members, one board member did not update the required background check within the required time frame.

Evidence:
During the licensing inspection, the licensing inspector observed the record for Board Member #2 contained a central registry finding dated 02/06/12. Background checks are required to be updated at least every three years from the date on the prior forms.

Plan of Correction: Per director - The central registry form has already been sent off and we are waiting for it to be returned.
