22VAC40-191 Background Checks (22VAC40-191)
The licensing inspector conducted an unannounced renewal inspection from 9:40am - 12:30pm. Upon arrival, there were seventy seven school age girls in care with eight staff. Sixty four girls and five staff were observed preparing to leave for a field trip to the wave pool. Thirteen girls and three staff remained at the center. The girls remaining at the center were observed at free play and during lunch. Four board officer's, three staff's and four children's records were reviewed. Standard #: 22VAC40-185-160-A Description: Based on record reviews, the center failed to have a new staff obtain a tuberculosis screening within 21 days of employment. Evidence: 1. The inspector reviewed three staff records. One of three records reviewed (staff record #1) did not contain a tuberculosis screening and staff's date of employment was 06/27/2016. Plan of Correction: The COO asked staff #1 to schedule a tuberculosis screening with their health care provider during the inspection. Staff #1 called and has an appointment on Monday, 8/1 for a tuberculosis screening. Staff records will be reviewed to ensure all contain current TB screenings.
Standard #: 22VAC40-185-60-A-7 Description: Based on record reviews, the center failed to obtain all information for the children's records. Evidence: 1. The inspector reviewed four children's records. Two of four records reviewed (children's records #2 and #3) listed allergies, but actions to take in an emergency situation were not addressed. Child record #2 listed allergies to tree nuts and child record #3 listed allergies to penicillin. Plan of Correction: The Chief Operating Officer (COO) will request this information from the children's parents today. The children's records will be reviewed to ensure all information is complete.
