Standard #: 22VAC40-185-240-D-4
Description: Based on record review, it was determined that children for whom emergency medications have been prescribed have not always been in the care of staff who completed an approved medication administration training (MAT) program. Evidence: 1. On the date of inspection the inspector reviewed two emergency medications belonging to children in care. 2. Staff #1 did not have a record of MAT training. 3. Staff #2 had a MAT training certificate which had expired 1/6/2014. 3. Staff #2 stated that she was aware her training had expired and has scheduled to take the training again.

Description: Based on observation the center did not keep hazardous substances in a locked place using a safe locking method that prevents access by children. Evidence: 1. In an unlocked cabinet, under the classroom sink used by children, the inspector found a box of wet floor mopping pads labeled "keep out of reach of children". 2. In an unlocked cabinet, above the classroom sink used by children, the inspector found a container of disposable disinfectant wipes, bleach scrub, and three bottles of spray cleaners.

Description: Based on observation, the center did not keep all medication in a locked place using a safe locking method that prevents access by children. Evidence: The inspector found a bottle of ibuprofen, for staff use, in an unlocked cabinet in the classroom used by children.

Description: Based on observation, the center did not ensure that food containers brought from home were clearly dated and labeled in a way that identifies the owner. Evidence: The inspector observed 12 unlabeled and undated lunch bags, on a cart outside of classroom, prior to meal service.
