Comments:
The licensing inspector conducted an unannounced renewal inspection Tuesday, July 26, 2016 from 10:05am to 11:30am. The children were observed playing outside, making Chinese paper lanterns, stringing beads and having sensory paly with shaving cream. A formal observation was completed with the school age class. The infants were observed having snack and then palying on the floor with toys. The lunch menu was posted and lunch was to consist of fish sticks, fries, macaroni and cheese, peas, apples, a roll and milk. Interviews were held with staff throughout the inspection. Additionally, when appropriate the inspector interacted with children in each classroom. All classrooms and playgrounds were inspected today as well as the facility?s van. Three children?s records and one employee record were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 6/27/2016 Last fire inspection: 7/14/2016 Last health inspection: 12/31/2015 Total enrollment today: 31 Today, the following child to staff ratios were observed: Infants and Toddlers ? 9:3 Preschool (Twos ? Three's)-8:1 School Age ?14:1 The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection. Standard #: 22VAC40-185-140-B
Description: Based on a review of children's records on 7/26/2016, the center did not ensure that the physical examination obtained in one of three records was obtained within the required time frame. EVIDENCE: The physical examination for Child #2, who is three years old, is dated 3/6/2014. Child #2's first day at the center was 4/4/2016. If the child has had a physical examination prior to attendance, it shall be within 12 months prior to attendance for children two years of age through five years of age.

Plan of Correction: Will check dates prior to enrollment to make sure within time frame.

Standard #: 22VAC40-185-280-B
Description: Based on observation on 7/26/2016, the center did not ensure that cleaning products were kept in a locked place. EVIDENCE: Cleaning supplies were observed in an unlocked closet in the rear of the school age classroom. Hazardous substances such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children.

Plan of Correction: Corrected on site-door locked. Will have opener staff to check to make sure locked.

Standard #: 22VAC40-185-290-3
Description: Based on observation on 7/26/2016, the center did not ensure that all electrical outlets had protective covers. EVIDENCE: There were three outlets without covers on the power strip in the infant room. The power strip was located out of the reach of the children. Electrical outlets shall have protective covers.

Plan of Correction: Will fix today. Will have opener staff to check to ensure that outlets have covers.

Standard #: 22VAC40-185-350-E-1
Description: Based on observation on 7/26/2016, the center did not ensure that the applicable ratio was followed. EVIDENCE: There were nine infants and toddlers with two staff members in the infant room from 10:45a.m. to 10:55a.m. For children from birth to the age of 16 months: one staff member for every four children.

Plan of Correction: Staff had an emergency and had to leave. Then a child that was not expected to come today came in. Will not send staff home until coverage comes in.
