Comments:
An unannounced renewal inspection was conducted today at 2pm. There were 21 (toddlers-5yrs.) directly supervised by 3 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping. There was an abundant supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-160-C
Description: Based on review, the facility failed to ensure that at least every two years from the date of the first initial screening or more frequently as recommended by a licensed physician or local health department. Evidence: Reviewed 4 staff records and found that Staff 1's last TB screening was on 7/17/14.

Plan of Correction: Staff person will have TB by August 15, 2016.

Standard #: 22VAC40-185-60-A
Description: Based on review, the facility failed to ensure that children's records were maintained with the required information. Evidence: Reviewed 4 children's records and found that CH1, CH3 and CH4 did not have documentation of parent's annual review of emergency contact information. CH1's last review was 3/4/15, CH3's last review was 6/24/14, and CH4's last review was on 3/11/15.

Plan of Correction: Records are updated each Aug - will be Aug 15.

Standard #: 22VAC40-185-550-D
Description: Based on review, the facility failed to conduct monthly practice evacuation drills. Evidence: Reviewed the practice evacuation drill log and found that the last documented drill was on 5/3/16.

Plan of Correction: Will be updated and monthly drills will be done.
