An unannounced monitoring inspection was conducted from 9:15 am-12:00. At the time of entrance, 54 children were in care with 12 staff members present. Children were observed during snack time, engaged in group activities, books, sequencing and outdoor splash play. A selection of staff and children records, medications and other documents were reviewed. The violations were reviewed with the center director. During this inspection, a investigation was conducted based on a self report incident. Staff were interviewed, there was no evidence to support the allegations. Standard #: 22VAC40-185-330-B Description: Based on physical observation, where playground equipment is provided, resilient surfacing shall comply with the minimum safety standards. Fall zones are defined as the area underneath and surrounding equipment. Evidence: The areas underneath and surrounding the climber on the large playgound requires 6 inches of surfacing. The ground was bare at the bottom of the 2 slides and 2 ladders and the pea gravel was about one inch in depth surrounding the structure. Plan of Correction: I will have someone rake and move the pea gravel under the climber.
Standard #: 22VAC40-185-500-B Description: Based on physical observation, disposable diapers shall be disposed in a leakproof system that is either foot operated or used in such a way that neither the staff members hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: The staff in the Montessori 2 and Infant program were using their hands to open the trash can to dispose of diapers. Plan of Correction: I immediately moved a step on trash can in to each of these rooms.
Standard #: 22VAC40-185-510-C Description: Based on medication review, the center did not follow their medication policy by allowing long term medications without written authorization from the child's physician and parent. Evidence: The physician's signature for child #1 epi pen was observed on a Fairfax County Public School's authorization form and not the center's authorization form. The authorization form for child #2's Zyrtec was signed by the parent on 4/1/16 but no written authorization from the physician. Plan of Correction: I will contact the parents to complete the correct form.
Standard #: 22VAC40-185-530-A Description: Based on record review and staff interview, there shall be a staff member trained in first aid and CPR in care who is on the premises during the center's hours of operation. Evidence: There is no staff with current certification in first aid and CPR during the hours of 7:00 am until 3:00 pm. The staff's certification expired in 5/20016. Plan of Correction: I will schedule a training for staff.
Standard #: 22VAC40-185-550-D Description: Based on record review, the center shall implement a minimum od two shelter in place drills per year. Evidence: The center has not completed a shelter in place drill twice in a year. The last drill practiced was 6/2/15. Plan of Correction: I will implement a shelter drill this month.
