Comments:
An unannounced renewal inspection was conducted on Wednesday, August 3, 2016, from approximately 10:15am until 1:30pm.
Present today were five infants with two staff creating finger painting art. Seven infants and two staff were on a walk. There were eleven two year old children with two staff playing outside and two children with one staff indoors. The three and four year old classroom had nine children playing outdoors with one staff and nine children indoors with one staff. This classroom was learning the letter "Yy" and discussing Brazil. The children had created toucans and designed an Indian elephant. The Pre-K classroom had fourteen children present with two staff discussing Brazil and soccer.
Lunch: Macaroni and cheese, green beans, sliced cucumbers and milk

Four children's records and five staff records were inspected.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of children's records, the center did not ensure each enrolled child had documentation of a physical examination within 30 days of attendance. Evidence: Out of four children's records inspected, Child #1started 12/14/15, Child #2 started 6/13/16 and Child #3 started 6/20/16. There is no documentation of a physical for these children.

Plan of Correction: Per Center: All physical examinations will be updated accordingly. Emails will be sent out to the above listed families for all documentation due no later then 8/18/16.

Standard #: 22VAC40-185-160-C
Description: Based on review of staff records, the center did not ensure staff submitted follow-up tuberculosis screening (TB) at least every two years from the date of the first initial screening or testing. Evidence: Out of five staff records inspected, Staff #1 TB expired 4/18/16; Staff #2 TB expired 4/18/16; Staff #3 TB expired 3/2/16.

Plan of Correction: Per Center: All TB tests will be updated accordingly. All TB tests will be signed, scanned and emailed to inspector as soon as possible. All forms will be sure to be with in compliance going forward.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on review of staff records, the center did not ensure each staff completed a sworn statement before three years since the dates of the last sworn statement. Evidence: Out of five staff records inspected, Staff #1 Sworn statement expired 2013; Staff #2 Sworn statement expired 4/18/13; Staff #3 Sworn statement expired 2014.

Plan of Correction: Per Center: All sworn statements will be updated accordingly. All updated sworn statements will be filled out, signed and dated and sent to Inspector 8/4/16.
