Standard 22 VAC 40-185-(8)-570.I requiring a one-day's emergency supply of bottles, nipples and commercial formula was discussed with the administrators. The use of disposable bottle liners was discussed.
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 12:25 PM and departed at 2:30 PM. Sixteen children were in care with four caregivers. Children were observed napping, during transition to afternoon activities. Afternoon snack service was observed. It was reported that no children have been administered any medication and that no medication is stored for emergency administration to the enrolled children. Three new employee records and a sampling of five children's records were reviewed. An exit meeting was conducted with the program director and facility administrators during which the facility's plans of correction for the violations cited were obtained. Standard #: 22VAC40-185-60-A Description: Based upon review of five children's records and staff interview, the facility has not ensured that each child's record includes the names and addresses for two persons to be contacted in an emergency when the parent cannot be reached. Evidence: 1. The record provided for child 1 does not include the complete street addresses for the two emergency contact persons. 2. Staff 1 and staff 4 were unable to provide the address for the two emergency contact persons for child 1. Plan of Correction: The facility responded with the following: The facility will immediately obtain the required information from the parent of child 1. Children's records will be audited to ensure that all information is present.
Standard #: 22VAC40-185-550-M Description: Based upon review of the written records of children's injuries, the facility has not ensured that all required information is documented. Evidence: 1. An injury record dated 6/20/16 for child 5 does not include the time the parent was notified of the child's injury. 2. An injury record dated 8/2/16 for child 6 does not include the date or time the parent was notified of the child's injury. Plan of Correction: The facility responded with the following: All staff will be reminded of the required information when documenting children's injuries. The administrator signing the report will check to ensure that all information is documented.
