- Technical Assistance:
Technical assistance was provided for the playground. We discussed the wooden equipment, swings, the tire activity were in need of repair. The facility has placed the large playground off limits until the modifications can be made. The facility will be using the fenced in playground.
An unannounced monitoring inspection was conducted with the facility director. There were thirty-one children in care, the age range was from 6 months to 12 years. Children were observed during art projects and outside playing carnival themed games and face painting. Seven children records and three staff records were reviewed. The inspection began at 11 AM to 2:30 PM.
Standard #: 22VAC40-185-130-A Description: Based on a review of children records not all of the files contained immunization records. Evidence: Seven children records were reviewed during the inspection. Child A with a start date of 6/6/2016, Child B with a start date of 5/31/2016, and Child C with a start date of 5/31/2016 did not have documentation of immunization. Plan of Correction: Facility will ask the parents for the missing immunization record.
Standard #: 22VAC40-185-140-A Description: Based review of children records the facility did not obtain documentation of physical examination within 30 days of enrollment. Evidence: Seven children records were reviewed during the inspection. Child A with a start date of 6/6/16, Child B with a start date of 5/31/2016, and Child C with a start date of 5/31/2016 were missing documentations of physical examinations. Plan of Correction: Facility will request the documentation of physical examinations from the parents.
Standard #: 22VAC40-185-60-A Description: Based on a review of children's records the facility did not to maintain complete records for all children. Evidence: Child A with an enrollment date of 6/6/2016 did not have proof of identity. Plan of Correction: Facility will request the proof of identity from the parent.
Standard #: 22VAC40-185-180-D Description: Based on review of staff records the facility failed to obtain disclosure documentation regarding moving violations that occurred five years prior to or during assignment as a driver. Evidence: Three staff files were reviewed. Staff A, Staff B, and Staff C records did not have documentation of driving statements. Plan of Correction: Facility will have the staff members fill out driving statement today. Facility will also order a copy of the staff driving records.
Standard #: 22VAC40-185-280-B Description: Based on observation hazardous substances were accessible to children. Evidence: In the 3/4s classroom there was a bottle of Glycerine stored in an unlocked closet which could be accessed by children. In the school age classroom there was a bottle of Glycerine stored on a counter top which could be accessed by children. In the pavilion there were 14 cans of spray paint in an unlocked closet which could be accessed by children. All labels stated to keep out of reach of children. Plan of Correction: All hazardous supplies utilized in the classroom will be locked. The facility will purchase and install a lock on the pavilion closet.
Standard #: 22VAC40-185-580-I Description: Based on observation field trip information was not posted and visible at the center site. Evidence: There was no information posted in the center that the school aged children were on a swimming trip on 7/15/2016. Plan of Correction: Facility will be posting the field trips and events in the center.
Standard #: 22VAC40-185-580-L Description: Based on observation permission for transportation and field trips was not secured before the scheduled activity. Evidence: There were no permission for children to be transported on the swimming field trip on 07/13/2016 and 7/15/2016. Plan of Correction: Facility will complete permission slips for specific fields before the scheduled activities.
