An unannounced monitoring inspection was completed on 8/11/16. There were 209 children present from three months through 12 years in 13 groupings with 22 staff directly supervising, one additional staff, and the director present. Eight children's records and five staff records were reviewed. The inspector arrived at the center at 9:45 am and departed at 3:00 pm. The children were observed during various classroom activities, during outside times, during water play outside, during lunch, during movie time for the school-age children and during nap. All infant activities were observed. There was discussion with the director about the following: requirements for staff who drive a vehicle transporting children (Standard 180-D), opt-out requirement for field trips, child's swimming skill for children who go swimming.
Standard #: 22VAC40-185-370-5 Description: REPEAT VIOLATION Based on observation, the center failed to ensure that awake infants not playing on the floor or ground a change in play space at least every 30 minutes or more often as determined by the individual infant's needs. Evidence: 1. A three month old infant was observed to be awake and in a bouncy seat at arrival into one of the infant rooms at 12:30 pm. The infant was still in the bouncy seat at 1:15 pm and was still awake. Infants are required to be provided a change in play space at least every 30 minutes. Plan of Correction: This was corrected during the inspection when the inspector reviewed the requirement to move infants at least every 30 minutes. The staff person moved the child to the floor. The director will post reminder signs in the infant rooms that children need to be moved when awake even if they are content.
Standard #: 22VAC40-185-420-E-1 Description: Based on record review in the infant classrooms, the center failed to ensure that the daily records for each infant in care had documentation of all required information. Evidence: 1. The infant daily records were reviewed for 14 infants in care. There were 10 bowel movements documented on the record and there was no description of these bowel movements as required. Plan of Correction: The director will add a space to document the description of bowel movements for infants to the infant daily reports. The director will monitor that the staff properly fill out the description of bowel movements on the infant daily reports.
Standard #: 22VAC40-185-500-A Description: Based on observation, the center failed to ensure that hand washing was practiced as required. Evidence: 1. A staff person was observed preparing to feed a five month old infant a bottle. The staff person sat down with the bottle and the child and had not washed her hands before beginning to feed the infant. The infant had not had her hands washed prior to eating. Plan of Correction: This was corrected during the inspection. When the inspector reminded the staff right before giving the bottle to the child, the staff person washed her hands and the infant's hands. The director will review the requirements with the substitute staff and post a reminder sign at the food service areas.
Standard #: 22VAC40-185-550-C Description: Based on observation, the center failed to ensure that emergency evacuation and shelter-in-place procedures were posted in a location conspicuous to staff and children on each floor of each building. Evidence: 1. The procedures for fire drills (emergency evacuation) and shelter-in-place were not posted with the emergency evacuation and shelter-in-place maps. Plan of Correction: The director will add the procedures to the maps.
Standard #: 22VAC40-185-550-E Description: Based on record review, the center failed to ensure that there was documentation of emergency drills as required. Evidence: 1. There was no documentation of any shelter-in-place drills for 2015. Two drills are required to be practiced each year. Plan of Correction: The director states that the shelter-in-place drills for 2015 were practiced but just not documented. The director will ensure these drills are documented in the future.
Standard #: 22VAC40-185-550-M Description: Based on record review, the center failed to ensure that the required documentation for injuries were made for eight of 33 reports reviewed for August 2016. Evidence: 1.Two injury reports dated 8/8/16 for two different children did not have documentation of the time that the parent was notified of the injury. Both reports indicated that a phone call was made to the parent to notify them and the time of the call was not documented. 2.Two injury reports dated 8/8/16 for the same child at two different times of day did not have documentation of the date, time and method of notification to the parent. The parent had signed both of the reports but there was no date or time that the parent had signed it. 3.Two injury reports for two different children dated 8/9/16 did not have documentation of the date, time, or method of notification to the parent. The parents had signed the report but there was no documentation of the date or time that the parents had signed the report. 4.Two injury reports for two different children dated 8/10/16 did not have two signatures. The staff who were present during the time of the injury had signed the reports but a second staff or the parent had not signed the reports. Plan of Correction: The director will re-train staff on documentation for injury reports. The director will review all injury reports before they are filed.
Standard #: 22VAC40-185-570-F Description: Based on observation, the center failed to ensure that heated formula was stirred or shaken and tested for temperature before serving to children. Evidence: 1. A staff person was observed taking an approximately four ounce bottle of formula out of the crock pot. The bottle was placed on the counter for less than a minute and then picked up by the staff to serve to the infant. The bottle was not stirred or shaken and not tested for temperature before feeding to the infant. The infant being fed was five months old. Plan of Correction: The director will review the procedures for feeding infants with the substitute staff. The director will post a notice about stirring or shaking and testing temperature above the crock pots in the infant rooms.
