22VAC40-185 SPECIAL CARE PROVISIONS AND EMERGENCIES.
An unannounced complaint investigation was conducted on July 26, 2016 and today due to a complaint received in the Valley Licensing Office July 22, 2016. Interviews were conducted and documentation reviewed regarding no written reports of injuries being provided parents and a lack of supervision. There was not a preponderance of evidence to support the allegations. The complaint is determined to be not valid. On this date the director advised that she and the board are reviewing the current policies and procedures handbook for staff and parents for any changes that might be needed. Standard #: 22VAC40-185-550-M Complaint related: No Description: Based on a review of injury reports the facility failed to maintain a written record of all required information. Evidence: 1. On 08/09/2016 the licensing inspector reviewed written injury reports. There was no written documentation of the date and time of the injury and there was no documentation of the time when the parents were notified. 2. On 08/18/2016 staff #1 confirmed the current injury report does not include documentation of the date and time of the injury nor is there documentation of the time when the parents were notified. Plan of Correction: We will review the injury report and include the date and time of the injury and the time when the parents are notified. I can do this today.
