Comments:
On 8/23/2016, a self-reported other visit took place at the center today regarding a child given the wrong medication. This visit took place in the morning between 8:45-11:30 there were 7 classrooms observed during this inspection. After interviewing all involved staff, violations were cited regarding this incident. Standard #: 22VAC40-185-240-D-4
Description: Based on observation, review of center medication policy and interviews with staff, emergency medication was found not in the care of a staff member who meets the MAT requirement. Evidence: On 8/22/16, a self-reported incident occurred at the Center when Child #1 was given the medication of another child (Child #2)with the same name. Child #1 & #2 with emergency medication were both not in the care of a staff member with who meets the requirements for MAT training.

Plan of Correction: Center Administration stated that a refresher trainer of the five rights of administering medication will be given to all staff.

Standard #: 22VAC40-185-340-A
Description: Based on interviews with staff, a review of the medication policy, center staff failed to ensure their care, protection, guidance when a child was given another child's asthma medication. Evidence: On 8/22/2016, a self-reported incident occurred when center staff failed to ensure care, protection and guidance, when Center staff gave Child #1, child #2's asthma medication. Staff #1 who is MAT trained failed to ensure that the correct name on the medication corresponded with the actual child receiving the medication. After administering the medication Child #2, came to the front office with Staff #4 and was told that Child #2 was in need of their asthma medication. At that time Staff #1 realized that the wrong medication was give to Child #1.

Plan of Correction: Center Administration stated that a refresher trainer of the five rights of administering medication will be given to all staff. New medications procedures will be put into place today to ensure that medication is given correctly and to the right person.
