Comments:
An unannounced monitoring inspection was conducted today from 10:15am to 1pm. there were 15 children in care ages 20 months through six years old with two staff, one volunteer and the center cook. Children were observed in teacher and self-directed activities including watching a movie, play choice, lunch, rest room routines, and nap. Violations regarding administration, physical plant and special services were cited and appear on the violation notice.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and interview the center failed to ensure that each child's record contains all required information. Evidence: 1 - The records for child 1 - 4 do not contain documentation of annual updated information. a. Child 1 has a documented date of enrollment as 6/3/15. b. Child 2 has a documented date of enrollment as 5/4/15. c. Child 3 has a documented date of enrollment as 6/9/15

Plan of Correction: Center management will obtained updated information from each family annually as required.

Standard #: 22VAC40-185-280-B
Description: Based on observation and interview the center failed to ensure that hazardous substances are kept in a locked place using a safe locking method. Evidence: 1 - There is spray bottle of bleach water solution in an unlocked cabinet, on the wall, in the cafeteria. 2 - Staff present confirmed that the bottle contained bleach water solution.

Plan of Correction: The cabinet will be locked during all hours operation when the bleach water is not in use.

Standard #: 22VAC40-185-560-F
Description: Based on observation, documentation review and interview the center failed to ensure that meals served follow USDA requirements. Evidence: 1 - During observation of lunch service the center cook used a regular place setting table spoon to scoop fruit and vegetables from a serving bowl onto the children's plates. 2 - The posted menu and menus available to the center cook do not include information about required portions of the food groups based on the age of the children being served. 3 - The center cook confirmed that she used a regular large/table spoon to scoop food onto the children's plats. 4 - The center cook stated that she unaware of the required USDA portions. 5 - There were not any measuring scoops available to ensure that each child received correct serving size.

Plan of Correction: Center management will ensure that proper measuring utensils are available for persons serving meals to children in care. Additionally, the required USDA portions will be posted in the kitchen so that it is visible in the food preparation area.
