Comments:
An unannounced "Other" inspection was conducted on August 4, 2016 in regards to a self-reported incident concerning supervision. The licensing inspector reviewed Administration and Staffing and Supervision. Interviews were conducted with staff, an observation of the facility was completed and documentation was obtained and reviewed. Areas of noncompliance that were not reported but found are identified on the Violation Notice.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, in one of two children's records, the licensee did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: During the inspection conducted on August 4, 2016, there was no documentation of a physical examination in the record for Child #2 (start date as indicated in the record 2/20/16).

Plan of Correction: Plan of correction is to get updated physicals for the children and put the updated physical in their files as soon as possible. I will prevent this from occurring by making sure parents turning in physicals as soon as they enroll their children
