An unannounced monitoring inspection was conducted on August 30, 2016. Staff reported 14 children are currently enrolled in the program. At the time of the inspection, 13 children were present with 2 staff members. The children were observed during the lunch meal. After lunch the children were observed during outdoor play. The children were interacting with one another and appeared to enjoy their play time. Dismissal procedures were observed at the end of the program day. A sample of 3 children's records was reviewed. Prescribed medications and written authorizations were reviewed for 3 children. Documentation of the center's practiced emergency drills, the annual fire and health inspections, partial staff records (2) and notice of liability insurance coverage was reviewed. The first aid kit was observed. Age-appropriate materials were available in the center for the children's use. Findings of the inspection were reviewed with Center Staff at exit. Standard #: 22VAC40-185-70-A Description: Based on documentation reviewed, the Center failed to maintain the name, address and telephone number of a person to be notified in an emergency for each staff member. Evidence: Emergency information was not available for review for one of three staff members. Staff #2 was observed working in the center as a substitute on the date of inspection. Plan of Correction: The staff will complete the information for retention at the center.
Standard #: 22VAC40-185-260-B Description: Based on reviewed documentation, the Center had not obtained annual approval from the health department for meeting requirements for water supply and sewage disposal. Evidence: Documentation of the environmental/sanitation inspection was dated 03/17/15. The reviewed food inspection (01/11/16) and food permit (05/31/17) is current. Plan of Correction: The local health department will be contacted and requested to complete the documentation for water and sewage.
Standard #: 22VAC40-185-550-E Description: Based on reviewed documentation, the Center failed to maintain a record of the dates of the emergency practice drills for one year. Evidence: The center did not have documentation available for conducted emergency drills for the months of April and May, 2016. Plan of Correction: Staff will ensure an evacuation drill is conducted at least monthly. Information will be documented and maintained in the center for at least one year.
