The licensing inspector conducted an unannounced monitoring inspection Monday, July 11, 2016 from 10:20am to 12:40pm. The children were observed playing with toys, playing outside and listening to a story. Lunch consisted of Saucy Beef Bake (a beef and pasta dish), corn, pears and milk. Interviews were held with staff throughout the inspection. Additionally, when appropriate the inspector interacted with children in each classroom. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Three children?s records and one employee record were reviewed during this inspection. Today, the following child to staff ratios were observed: Infants & Toddlers ?7:2 Twos ? 15:2 Threes ?10:1 Pre-K ?10:1 School Age ?16:1 The violations from the previous inspections were checked for corrections. There was one repeat violation found during today?s inspection. If you have any questions about this inspection, please contact the licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Standard #: 22VAC40-185-330-B Description: Based on observation on 07/11/2016, the center did not ensure that the fall zones for playground equipment did not include barriers. EVIDENCE: The rocket ship climbing equipment on the preschool playground was located in the fall zone of the swing set on the same playground. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Falls zones shall not include barriers for resilient surfacing. EVIDENCE: A bottle of formula belonging to Child #1 did not have a date. Prepared infant formula shall be refrigerated, dated and labeled with the child's name. Standard #: 22VAC40-191-40-D-1-C Description: Based on a review of staff records on 7/11/2016, the center did not ensure that the staff record reviewed contained updated background checks. EVIDENCE: 1. The most recent criminal history check in the record of Staff #1 is dated 3/21/2013. 2. The most recent sworn statement in the record of Staff #1 is dated 3/5/2013. Any employee who is required to have background checks shall have a sworn statement or affirmation, and criminal history record check before three years since the dates of the last sworn statement or affirmation, and most recent criminal history record check report.
