22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring visit was done today August 30, 2017 beginning at 2:50 PM and ending at 5:15 PM. There were seventeen children present today. The children were observed during snack, discussion of weekend activities and homework time. They also played outside on the blacktop and play equipment to the rear of the school. The medications were reviewed today as well as a sample of children's files. The staff files were reviewed earlier today at the main office. The violations were discussed today with the child care director as well as the site coordinator. Corrective action was obtained during the discussion. The inspector and site coordinator also discussed the the front play equipment as it is off limits to the three and four year old children due to not enough resilient surfacing in the fall zones. The three and four year old children have been using the black top area only when playing outside. If you have any other questions, please contact the inspector at 540-309-2835.
Standard #: 22VAC40-185-60-A Description: Based on observation the center did not have all of the required information on the children's enrollment form. Evidence: 1. During the inspection the inspector reviewed four children's files and all of the files # 1-4 were missing information regarding: parent's place of employment/address and names of any additional schools attending. These questions had been left off of the enrollment form. Plan of Correction: The site coordinator will relay this information tho the director to have the form changed. Information will be obtained from the parents and documented.
Standard #: 22VAC40-185-190-A Description: Based on observation the center did not provide the required documentation to verify the qualifications of the named program director. Evidence: 1. During the inspection the inspector reviewed two staff files and staff # 1 is named a site coordinator and there was no documentation to verify that this staff person met the qualifications. Education, programmatic and management experience were not provided. Plan of Correction: The director of day care indicated she would obtain this information and then ensure the person is qualified for the site coordinator position.
Standard #: 22VAC40-185-340-F Description: Based on observation the center did not ensure that all children under the age of ten were within sight and sound supervision at all times. Evidence: 1. During the inspection the inspector passed a child in the hallway walking alone towards the cafeteria. When the inspector returned to the cafeteria the same child was in the Y program group of children. The inspector discussed this with staff # 1 & 2 and it was learned that child # 5 had left the cafeteria without asking staff to accompany him to the bathroom so he could change clothes for ball practice. The child was verified to be age nine (3/07) from his file review. In discussing the situation, all staff were in the cafeteria however, staff # 1 was exiting the inspection with the inspector and staff # 2 & 3 were at tables working with the children's homework. Through discussion with the staff it is estimated the child was out of sight and sound supervision for five minutes or less. Plan of Correction: The staff promtly discussed this with the child and will review the rules again with the children regarding supervision as it is different from being in school.
Standard #: 63.2-1721-D Description: Based on observation the center allowed a staff person to supervise a child alone without a criminal history check. Evidence: 1. During the inspection staff # 3 who was hired 8/29/16, was observed by the inspector taking a child to get an ice pack. The staff member and child were not accompanied by any other staff. Plan of Correction: The coodinator will ensure she and staff # 2 supervise the children together until the state police check is returned.
