- Technical Assistance:
1. Reviewed cot and crib spacing requirements.
Conducted an unannounced monitoring inspection 12:55-2:10pm. Observed 74 children + 15 direct-care staff. Ratios and supervision were in compliance. Children were napping and having quiet time during the inspection. Lunch served today: teryiaki chicken, oriental vegetables, peaches, bread/butter, and milk. Classrooms were found to be clean and well supplied with toys and equipment for the children. Required documents were posted. Record keeping and physical plant violations found. Standard #: 22VAC40-185-270-A Description: Based on observations made, it was determined that an area of the center was not maintained in a safe condition. Evidence: A wooden spindle on a gate on the playground was splintered/cracked and exposed about 1" of a nail. The nail was within reach of children. Plan of Correction: Placed work order with FDIC maintenance.
Standard #: 22VAC40-185-280-B Description: Based on observations made, it was determined that a hazardous substance was not kept in a locked place. Evidence: A bleach solution was found in an unlocked cabinet in the Toddler I classroom. Plan of Correction: Corrected on site. Teachers reminded that bleach cabinet must remain locked.
Standard #: 22VAC40-185-290-3 Description: Based on observations made, it was determined that an electrical outlet in the main hallway did not have a protective cover. Plan of Correction: Corrected on site. Teachers/FDIC cleaning crew reminded to replace covers.
Standard #: 22VAC40-185-440-E Description: Based on observations made, it was determined that there was not at least 12" of space between cots, or on at least one side when the cot was between furniture. Evidence: Three cots in the Two's-2 classroom had between 4"-8" of space between them; and two cots in the Four's classroom had not space between them, or between walls/furniture. Plan of Correction: Staff reminded of requirement. Change nap time room arrangement.
Standard #: 22VAC40-185-450-A Description: Based on observations made, it was determined that bottom linens were not provided for all cots. Evidence: Four children were observed lying directly on the mesh cot surface, as there was no bottom linen/sheet. Plan of Correction: Additional sheets ordered in case parents forget to bring from home.
Standard #: 22VAC40-185-510-J Description: Based on observations made, it was determined that a medication was not kept in a locked place. Evidence: A medicine belonging to staff was found in an unlocked cabinet in the Infant 2 classroom. Plan of Correction: Corrected on site. Staff reminded to keep personal items locked.
Standard #: 22VAC40-185-510-N Description: Based on records reviewed, it was determined that a medication on-site was not discarded 14 days from the expiration of the authorization. Evidence: The medication authorization for a medicine belonging to Child #1 expired on 7/22/16, and the medicine was not sent home or discarded after 14 days. Plan of Correction: Medication discarded. Running log of medication expirations dates created.
Standard #: 22VAC40-185-520-C Description: Based on observations made, it was determined that diaper ointments/creams were not labeled with the child's name. Evidence: One cream in the Infant 2 classroom was not labeled, and three creams in the Toddler I classroom were not labeled. Plan of Correction: Corrected on site. Teachers reminded to label all items and re-label as needed.
Standard #: 22VAC40-185-550-E Description: Based on records reviewed, it was determined that there was no record of a monthly emergency evacuation drill for February 2016. Plan of Correction: Yearly drill schedule reviewed to ensure one drill each month scheduled.
