Comments:
This unannounced renewal inspection was conducted from 9:45 A.M. until 12:30 P.M. During the inspection 32 children were in care with six staff members. The sample size consisted of six staff's records, three board officer's records, and three children's records. There was no medication at the center at the time of the inspection. Each classroom and outdoor play area was inspected. Children and staff were observed during singing, lunch preparations, hand washing, and snack. Staff were observed during transitions and behavioral management. The violation found today can be found on the attached violation notice. Standard #: 22VAC40-191-40-D-1-C
Description: Based on record review and interview, four out of nine staff did not have completed sworn statement or affirmation, search of central registry (CPS) and criminal history (CRC) record checks before three years since the dates the previous documents were completed. Evidence: 1. The date of the last CRC for staffs 3, 4, and 5 was 8/6/13. Documentation in the records show they were not mailed until some time in August. 2. The date of the last CRC for staff 6 was dated 6/12/13, the last CPS was 6/24/13, and the last sworn statement was 7/1/13. 3. Staff 6 stated the CPS and CRC for herself had not been processed for mailing yet.

Plan of Correction: Records will be reviewed on a quarterly basis to determine which employees are due for updated paperwork. The Business Manger will obtain updated paperwork and mail prior to 60 days of expiration on the current paperwork to allow for processing time. CRC, CPS and sworn statement for Staff 6 was obtained and mailed on August 25, 2016.
