Standard #: 22VAC40-185-340-A Description: Based on staff interviews, staff failed to ensure that when supervising children, they shall always ensure their care, protection and guidance. Evidence: on 6/7/16 3 staff members 1, 2 and 3 left a child alone in the classroom, while escorting 18 other children to the playground. The child was left alone for approximately 10-15 minutes, until a parent entered the room and found the child. Plan of Correction: The staff members involved were retrained on proper procedures for leaving a classroom-counting and comparing number to sign in/out sheet. New checklist implemented, requiring staff to write down number of children and staff before leaving and upon return.

Evidence: On 6/7/16 a child was left in the bathroom of the classroom while 18 other children and 3 staff members (staff 1, 2 and 3) left the room to walk to the playground down the street. The child was alone in the classroom and unsupervised for approximately 10-15 minutes. A parent that was visiting the class to pickup their child's belongings found the child, and took the child to the main office across the street. Plan of Correction: The staff members involved were retrained on proper procedures for leaving a classroom-counting and comparing number to sign in/out sheet. New checklist implemented, requiring staff to write down number of children and staff before leaving and upon return.
