Comments:
The licensing inspector conducted an unannounced Other inspection in response to a self reported incident that was received by the licensing office on 6/7/2016. Interviews were conducted with staff relating to allegations lack of supervision, when a child was left unsupervised in the classroom for approximately 10-15 minutes. The information gathered during the investigation supports the allegations, so the incident is determined to be valid. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 calendar days of receipt. Standard #: 22VAC40-185-340-A
Description: Based on staff interviews, staff failed to ensure that when supervising children, they shall always ensure their care, protection and guidance. Evidence: on 6/7/16 3 staff members 1, 2 and 3 left a child alone in the classroom, while escorting 18 other children to the playground. The child was left alone for approximately 10-15 minutes, until a parent entered the room and found the child.

Plan of Correction: The staff members involved were retrained on proper procedures for leaving a classroom-counting and comparing number to sign in/out sheet. New checklist implemented, requiring staff to write down number of children and staff before leaving and upon return.

Description: Based on staff interviews, staff failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision. Evidence: On 6/7/16 a child was left in the bathroom of the classroom while 18 other children and 3 staff members (staff 1, 2 and 3) left the room to walk to the playground down the street. The child was alone in the classroom and unsupervised for approximately 10-15 minutes. A parent that was visiting the class to pickup their child's belongings found the child, and took the child to the main office across the street.

Plan of Correction: The staff members involved were retrained on proper procedures for leaving a classroom-counting and comparing number to sign in/out sheet. New checklist implemented, requiring staff to write down number of children and staff before leaving and upon return.
