Comments:
An unannounced renewal inspection was conducted on September 13, 2016. There were 117 children present at arrival to the facility in eight classrooms with seventeen staff members. Additional staff members and volunteers were on site for administrative and support purposes. The children were observed having lunch, napping, hand washing, and reading books and quiet activities. The outdoor playground equipment is not being utilized at this time. Five children's records were reviewed. Five medications were reviewed.

Violations:
Standard #: 22VAC40-185-320-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that each restroom area provided for children was equipped with all necessary supplies. Evidence: During the inspection conducted on September 13, one of two stalls in the boys bathroom located in the hallway was not equipped with toilet paper.

Plan of Correction: Spoke with custodians of the center. The toilet paper holder is being repaired.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and inspection of the facility, the licensee did not ensure that the findings of the most recent inspection of the facility were posted. Evidence: During the inspection conducted on September 13, 2016, the findings of the most recent inspection conducted on June 17, 2016 were not posted. The findings from the 4/20/16 inspection were posted.

Plan of Correction: Already spoke to Human Resources Representative and they will forward a copy of the findings. I will make sure that the most recent inspection will be posted.
