Technical Assistance:
Consultation was given on the following: 1. Please have parents initial any food sensitivity/preference/allergy that is posted or place a cover sheet over the names to protect confidentiality. 2. Proof of identity can be verified with a birth letter from the hospital, birth certificate or a passport. 3. On walking field trips the staff should bring emergency contact information for children, a cell phone, first aid kit and any emergency medication a child may need. If medication is present, a MAT trained staff member should accompany the walkers. 4. All lines on the child record form should be completed. If not applicable have the parent write n/a, none or draw a line through the question.

Comments:
An unannounced monitoring visit was conducted during the Conditional licensure period. Four classrooms were in operation (18 children) with direct care staff. Staff ratios were high as not all enrolled children have attended since the first day of school. Observed children in free play in large room, infant feeding and staff reading to the baby, singing songs and transition to "Kids Cafe" for lunch. Lunch served today was pizza dippers, steamed carrots, fresh fruit and milk. No medication was reported to be on=site. Three staff currently have Medication Administration Training (MAT) certification. Rooms are well organized and supplied with ample materials and equipment. Two children's files were reviewed.
