- Technical Assistance:
Discussion held with director about updating current year forms with a space for parent employment information. Information was obtained during inspection from prior year records. Discussion held with director on the topic of background checks that may take longer than 30 days to return from processing and documentation required when following up with agencies that process the search documents.
An unannounced monitoring inspection was conducted on 09/16/2016 from 10:00am 11:45 am. The inspector observed 4 classrooms with a total of 59 children and 9 supervising staff, within the supervision guidelines. The children were observed playing outside, participating in music class, center time, art projects and conducting smooth transitions. Positive interactions were observed in all classrooms during this inspection. A review of staff and student records, medications, physical plant and playground was conducted today.. Standard #: 22VAC40-185-70-A Description: Based on record review, not all staff records contained all the required documentation. Evidence: 1. Staff #1's record did not contain a Sworn Disclosure Statement. 2. Director has documentation that the form has been completed but could not produce the document on the date of inspection. 3. Site director has mailed the document to be processed with another background check instead of keeping the document on site. Plan of Correction: Director will have the staff member complete a new document of 9/20/16 at the beginning of her next scheduled shift.
Standard #: 22VAC40-185-280-B Description: Based on observation, not all hazardous substances were kept in kept in a locked place using a safe locking method that prevents access by children. Evidence: A bottle of water conditioner labeled "Keep out of reach of children" was observed in an upper unlocked cabinet in the Ducklings/Bunnies classroom. Plan of Correction: Corrected during inspection.
