- Technical Assistance:
The licensing inspector conducted an unannounced monitoring inspection August 15, 2016 from approximately 10:15am to 1:45pm. The following ratios were observed with a total of 45 children present during the inspection: Infants - 5 children with 2 staff Ones - 5 children with 2 staff (during nap time 5 children with 1 staff) Twos - 12 children with 2 staff Threes - 10 children with 2 staff Fours - 5 children with 1 staff Fours - 8 children with 1 staff School Age children were not present during the inspection. During the inspection, the Infants were observed having their diapers changes, being held and fed and sleeping in their cribs. The Ones were observed playing with toys, having their diapers changed and blowing bubbles. The Twos classroom was observed playing outside, taking bathroom breaks and washing their hands. The Threes classroom was observed playing outside, eating lunch and taking bathroom breaks and washing their hands. The Fours classrooms were observed having stories read to them and eating lunch. Staff was observed interacting with the children and providing assistance when needed. Lunch consisted of corn dog nuggets, tater tots, corn, mandarin oranges and milk. Later children began to transition into nap time. All areas of the center including classrooms, hallways, bathrooms, kitchen, and playgrounds were inspected. The center was equipped with toys and supplies and items were available to the children. The following information was reviewed: emergency supplies, injury reports, fire and shelter-in-place drills, daily attendance, daily sheets, parent agreements, and staff training information. The owner stated medications were not present at the facility at this time. Transportation is provided. The vans and transportation documents were reviewed during the inspection. Standard #: 22VAC40-185-270-A Description: Based on observations, the facility did not ensure all areas and equipment were maintained in a safe and operable condition. Evidence: During the inspection, the licensing inspector observed the following: 1. A plastic house located on the playground had a crack in the roof and side of the house. The roof of the house was collapsing. 2. A half wall located in the Twos' classroom had a shelf on top of it that contained areas of peeling paint approximately two to three inches long. 3. A ceiling tile located in the Threes' classroom contained brown stains and dark greenish substances that appeared to be mold or mildew. Plan of Correction: Per owner : 1. The plastic house was removed from the facility during the inspection. 2. The shelf in the Twos' classroom will be sanded at the end of the day. 3. A replacement ceiling tile has already been ordered and will be replaced.
Standard #: 22VAC40-185-350-E-1 Description: Based on observation, the facility did not ensure the required staff to child ratio for children birth to 16 months of age was being maintained. Evidence: During the inspection, the licensing inspector observed five children being supervised by one staff member in the Ones classroom at approximately 1:00pm. The youngest child in the classroom was 13 months of age. The children were observed sleeping on mats in the classroom. The infant ratio is not allowed to be doubled at nap time and still required one staff member for every four children that are birth to 16 months of age. Plan of Correction: Per owner - We will make sure we do not double the staff to child ratio during naptime for any classroom that has children under the age of 16 months.
Standard #: 22VAC40-191-60-C-1 Description: Based on review of four staff records, one record did not contain a criminal history record report obtained within 30 days of beginning employment. Evidence: During the inspection, the licensing inspector observed the record for Staff #3 (hire date 06/07/16) contained a criminal history record dated 08/10/16. Criminal history records are required to be obtained within the first 30 days of staff beginning employment. Plan of Correction: Per owner - We will make sure that we will follow up with the correct agencies regarding background checks and document those conversations.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of four staff records, one record did not contain a central registry finding within 30 days of beginning employment. Evidence: During the inspection, the licensing inspector observed the record for Staff #3 (hire date 06/07/16) did not contain a central registry finding. Central registry findings are required to be obtained within the first 30 days of staff beginning employment. Plan of Correction: Per owner - We will make sure that we will follow up with the correct agencies regarding background checks and document those conversations.
