Comments:
An unannounced renewal inspection was conducted on September 13th 2016 between 9:20am and 11:35am. There were 33 children in the direct care of 8 staff members. Children were observed working on colors, shapes, days of the week, months of the year, and an art project. Children also had a special visitor come to the center and read to them. Staff were consistently engaged with the children. A sampling of staff and children's records were reviewed. The first aid kit, fire inspection, health inspection, emergency supplies, physical space, and supervision were also observed and inspected. Standard #: 22VAC40-185-130-B
Description: Based on record review, 1 of 3 records reviewed did not have additional immunization documentation every six months for children under the age of two years. Evidence: Child #1 was born on 12/31/2014 and the last documented record of immunization was dated 1/8/2016

Plan of Correction: Will inform the parents today and will get it via fax.

Standard #: 22VAC40-185-70-A
Description: Based on record review, 2 of 2 staff records did not contain the required documentation. Evidence: Staff #1 and Staff #2 do not have complete emergency contact information, including phone number and address, on file.

Plan of Correction: Will complete today.

Standard #: 22VAC40-185-280-B
Description: Based on observation, a disinfectant spray was not in a locked place using a safe locking method. Evidence: A disinfectant odor spray was in a green cubby in the primary Montessori classroom.

Plan of Correction: Was corrected at the time of inspection.

Standard #: 22VAC40-185-430-M
Description: Based on observation, the facility had recalled item in a classroom. Evidence: An item in the infant's room was recalled on August 15, 2012.

Plan of Correction: It was removed from the classroom during inspection.
