Comments:
Thank you for your assistance during this unannounced renewal inspection. This on-site visit was conducted on September 16, 2016 from 11:00 a.m. through 2:00 p.m. On this date, there were eighty-three (83) children in the care of eleven (11) staff members in eight classrooms. There are a total of one-hundred nine (109) children enrolled in this program. Five (5) children's records were reviewed. Fourteen (14) staff records were reviewed and updated on August 11, 2016 from 12:30 p.m. through 3:00 p.m. The medication and medication paperwork was reviewed for two children. Areas of non-compliance may be found on the violation notice. Risk scores were discussed. Areas observed were story time, educational activities, free choice activity, outside play, infant feeding, lunch, hand washing/diapering and bathroom procedures and nap. All interactions between staff and the children were appropriate and the children seemed comfortable with the daily routine. There are a wide variety of age/stage materials for the children in care. Description: An air freshener plug-in was in one receptacle in a bathroom used by children. Evidence: Based on observation and conversation with the director, it was confirmed the air freshener was plugged into an outlet in the children's bathroom.

Plan of Correction: The air freshener plug-in was removed and a protective covering was placed in the electrical outlet.

Standard #: 22VAC40-191-40-D-1-B
Description: Staff A did not have documentation of obtaining a central registry finding report within 30 days of employment. Evidence: Based on record review conducted on August 11, 2016 and discussion with the director, it was confirmed Staff A had not obtained a central registry finding report.

Plan of Correction: The licensing representative reviewed the central registry finding today and it was dated 8/12/2016.

Standard #: 22VAC40-191-40-D-1-C
Description: One administrative staff member had not obtained an updated criminal history check and a central registry finding. Staff A had not obtained an updated central registry finding. Evidence: Based on record review conducted on August 11, 2016 and conversation with the director, it was confirmed a criminal history check and central registry finding had not been updated for one administrative staff member. Last documented criminal history check was dated 02/09/2013. Last documented central registry finding was documented 02/09/2013. Staff A last documented central registry finding was dated 1/17/2013.

Plan of Correction: The criminal history check and central registry findings were completed and mailed on August 12, 2016. Updates will be done every three years.
