An unannounced complaint investigation was initiated today at 10:30am. This was in response to an email the Licensing Inspector received on September 12. The complaint included concerns with the center following it's policies and obtaining health information on children before the began care at the center. After records were reviewed and interviews were conducted, this complaint was determined to be "valid". The center did not obtain all health forms prior to children attending the center, identification was not requested for all new parents by all staff and the sign in/out procedures were not being followed by parents every day their child was in care. The director will be making changes to the Parent handbook and training of staff on these responsibilities. Should you have any questions, please contact me at email@example.com.
Standard #: 22VAC40-185-130-A Complaint related: Yes Description: Based on file review, 1 out of 14 children's records do not have an immunization record on file before receiving care at the center. Evidence 1. Child #8 began receiving care on August 18th. There is no immunization record on file. Plan of Correction: The parent is supposed to bring this the next day the child is in care.
Standard #: 22VAC40-185-40-E Complaint related: Yes Description: Based on interview and observations, the center did not comply with all of their written policies. Evidence 1. The parent handbook states "Parents are requested to submit all the relevant information listed below prior to the child's first day at the school." Included below is "Physical and Immunization Form (Health Form)". The director stated that child #14 did not bring in the physical examination prior to the first day in care. 2. The parent handbook states "State regulation require parent to sign their children in and out every day." On 9/12, child #14 received care at the center. The child was signed in at 9:10am. However, there was no documented time recorded on the parent sign in/out sheet for when the child was picked up from the center. Additional documentation was provided, in which the teachers document the time that each child is picked up daily. 3. "We always request identification from unfamiliar people, verifying that their picture match their face and their name." Based on interview, it was determined that not all staff ask for identification from parents whom they have not met. Plan of Correction: The director will be working on making changes to the Parent Handbook. These changes include: Having staff sign the children in and out of the center daily, and abiding by the state regulations in obtaining to the Physical examination within the allowed 30 days after the child begins care at the center The teachers will be reminded that they must ask all parents who pick up a child from the center for identification.
