Description: Based on record review and staff interviews, the program has not updated the injury prevention procedures in the last year.

Plan of Correction: We have been working on the updated plan after finishing the review of last year's accident reports.

Standard #: 22VAC40-185-60-A
Description: Based on record review of five children's files, the center has not obtained yearly updates for two of the files. The files do not include parent's signatures to verify enrollment information is current.

Plan of Correction: We have sent out request for updates and have been working to get all of the files updated.

Standard #: 22VAC40-185-550-A
Description: The center has not updated the emergency preparedness plan as necessary to document the changes in staff responsibilities. The center has a new Supervisor and contact names and numbers should reflect the changes.

Plan of Correction: We are currently also updating that information.
