Original MAT must be kept on site.
An unannounced monitoring inspection was conducted today with the director, who was present with 3 additional staff and 31 children. The children were engaged in homework time during most of the inspection. For snack, they were provided cereal and milk. Two children had medications on site. Four staff and children's files were reviewed. Documentation of injuries/accidents were reviewed as well. Health inspection: 5/3/16 Fire inspection: 10/21/15 Arrival time: 4:35 p.m. Departure time: 6:15 p.m.
Standard #: 22VAC40-185-240-D-1 Description: Based on a review of four staff files and an interview with the director, it was determined that the original l Medication Adminsitration Training (MAT) certificate was not onsite. Evidence: Two children had emergency medication on site and the MAT certificate was a copy and not the original document. Plan of Correction: I have the original MAT certificate and I will place in the file 9/22/16.
Standard #: 22VAC40-185-510-C Description: Based on a review of medications, it was determined that there was no written authorization to administer a medication for one child. Evidence: There was no authorization to administer Proair Inhaler for child A. Plan of Correction: The form was signed by the physician and parent but the actual name of the medication was not listed on the form. I will obtain this information from the parent.
