Comments:
An unannounced renewal inspection was conducted with the director beginning at 1:30 PM and ending at 4:10 PM. There were twenty children in care ages ranging from seven months to five years old. The children were observed during nap time. Six children's records were reviewed. Three staff records were reviewed and determined to be complete. Seven medications were reviewed. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent fire inspection was conducted on August 11, 2015. The most recent health inspection was conducted on March 24, 2016. The center has eighteen staff members that are CPR and 1st aid certified. There are three staff members with current Medication Administration Training (MAT) certification.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of six children's records it was determined that the center did not maintain complete records for all children. Evidence: Child B's record was missing the work phone number for one parent. Child C and D's record were missing the address for one emergency contact.

Plan of Correction: All information will be obtained.

Standard #: 22VAC40-185-80-A
Description: Based on review of the attendance record, the facility did not ensure all children were included on the written attendance record. Evidence: There were two children present in the infant classroom but none marked present on the attendance sheet. The attendance sheet in the two year old classroom had four children marked present on the attendance sheet but there were six children present.

Plan of Correction: The director will meet with the staff regarding attendance record keeping.

Standard #: 22VAC40-185-260-A
Description: Based on a review of documentation it was determined the facility did not have a current annual fire inspection. Evidence: The most recent fire inspection was dated August 11, 2015.

Plan of Correction: Fire Marshal was contacted during the inspection and an inspection has been scheduled for September 29, 2016.

Standard #: 22VAC40-185-270-A
Description: Based on observation not all areas were maintained in a safe and operable condition. Evidence: The doors in the two year old classroom leading to the outside play area used as emergency exits sticks and requires substantial pushing and pulling force to open and close. Six of the eight S-hooks on the swings were overlapping posing a strangulation hazard. The swing set was missing eleven out of twelve lock nuts for the bolts that secures the swings to the top bar. On the large playground there were two metal drainage pipes with exposed sharp edges.

Plan of Correction: The swings are of limits until repairs are completed. A contractor will replace all materials from the top down. The classroom doors will be repaired and the drainage pipes will be covered.

Standard #: 22VAC40-185-510-G
Description: Based on review not all the medications had a written authorization from a parent or guardian to administer. Evidence: The medication authorization form for Child A's medication was dated August 10, 2016 and had expired on August 24, 2016.

Plan of Correction: The parent will be asked to complete a new medication form.
