An unannounced monitoring inspection was initiated today at 1:30am. The physical plant, 4 staff files, 4 children's files, annual fire and health inspection, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected today. There were 94 children under the supervision of 15 staff. During the inspection, children were observed bring fed and having diapers changed (infants), listening to music and resting quietly during nap time. For lunch today, the children were served whole wheat ziti with chicken, sausage, tomato sauce and baby kale, along with salad, cantaloupe and milk. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 6 staff certified in MAT (medication administration training). Standard #: 22VAC40-185-160-C Description: Based on file review, 1 out of 4 staff files does not have a TB test/screening completed within the past two years. Evidence 1. Staff #2 's most recent TB test/screeening was completed on 5/26/14. Plan of Correction: This will be completed again.
Standard #: 22VAC40-185-70-A Description: Based on file review, 2 out of 4 staff records do not have all required documentation. Evidence 1. Staff #3 and #4 do not have the following information on file: - Name address and and telephone number of a person to be notified in an emergency, which shall be kept at the center - Written information to demonstrate that the individual possesses the orientation training required by the job position. - Information, to be kept at the center, about any health problems which may interfere with fulfilling the job responsibilities. Plan of Correction: The director will obtain all of this and keep it on file.
Standard #: 22VAC40-185-570-E Description: Based on observations, there were infant bottles that were not labeled with all required information. Evidence 1. Children #1 and #2's bottles in the Infant room 4 did not have the date labeled on them. 2. Child #3's bottles in the Infant room 3 did not have the date labeled on them. Plan of Correction: Teachers were reminded of this during the inspection.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation, the findings of the most recent inspection of the facility were not visibly posted. Evidence 1. In the area where inspections are posted, the Inspection Summary (IS)was posted over top of the Violation Notice. This was previously cited on 7/14/15. Plan of Correction: Today's VN will be visibly posted.
