9/26/16 Arrived 8:15am Departed 12:15pm An unannounced renewal study was conducted today. Five infants were present with two staff. Two toddlers were present with one staff member. Seven twos were present with two staff. Seven threes and fours were present with one staff member. The children were eating breakfast, playing in centers, taking a nature walk, eating a snack and lunch and watching a movie. Standard #: 22VAC40-185-330-B Description: Based on a tour of the playground, the facility did not maintain resilient surface material under all equipment with moving parts. Evidence: 1. There is hard packed dirt beneath the tea cup ride. Plan of Correction: The director will request that maintenance personnel rake the pea gravel to insure that there is 6 inches under each piece of equipment which requires it.
Standard #: 22VAC40-185-510-J Description: Based on a tour of Club 88, the facility did not lock all medication. Evidence: 1. There is an Epi-Pen Jr in the large drawer of the teacher's desk. It is not locked. Plan of Correction: The director will remove the Epi-pen and contact the former parent to pick up the medicine. If they do not then the director will discard it. All medication will be kept in a locked area.
Standard #: 22VAC40-185-520-D Description: Based on a tour of the preschool room, the facility did not secure all insect repellent. Evidence: 1. There was a spray bottle of OFF on a shelf in a child's cubby. It was accessible to children. Plan of Correction: The teacher removed it from the cubby and placed it in an area that is not accessible to children.
Standard #: 22VAC40-185-530-A Description: Based on a review of documentation and an interview with staff, the facility failed to provide a staff member trained in first aid and CPR on duty at all times. 1. There was no one on duty from June 12 through September 20 who held current first aid and CPR certification. Plan of Correction: The director reported that all staff were trained in first aid and CPR on September 19 & 20.
Standard #: 22VAC40-185-560-M Description: During breakfast in the twos' room, the facility failed to require staff to sit with children during meals. Evidence: 1. The staff did not sit with children during breakfast. Plan of Correction: The director will insure that all staff are reminded to sit with children during meals.
