Today's visit was a routine monitoring visit. The inspection began at 2:15pm and concluded at 4:30pm and a a second inspection was conducted at the agency's main location on 9/27 in order to review staff records. During the inspection arrival of the children was observed, snack time was observed, homework time was observed, four out of sixty-one children's records were reviewed, emergency supplies were viewed, all four staff records were reviewed, and some policies and procedures were reviewed. Standard #: 22VAC40-185-160-C Description: Based on the inspector's review of all four staff records, the program failed to ensure a TB screening was no more than two years old. Evidence: The record for staff member #2 is dated 9/10/07 and is more than two years old. Plan of Correction: The staff member has completed a TB screening an will provide a copy to the program within a week of today's inspection.
Standard #: 22VAC40-185-40-G Description: Based on the inspector's review of four out of sixty-one children's records, the program failed to ensure there was documentation of viewing proof of identity for two out of four records reviewed. Evidence: The records for children #3 and #4 did not have documentation of viewing the proof of identity for each of the children. Plan of Correction: The Program Director will ask the parent to bring in appropriate proof of identity and it will be documented in the children's records.
Standard #: 22VAC40-185-60-A Description: Based on the inspector's review of a sample of four out of sixty-one children's records, the program failed to ensure two children's records contained all of the required information. Evidence: The records for child #3 and #4 did not include the physical address for one out of two emergency contacts. Plan of Correction: The Program Director will request the information from the parents and add it to the records. Additional children's records will be audited for completeness.
