- Technical Assistance:
An exit interview was conducted with the owner to discuss the results of today's inspection. Consultation was given on upcoming subsidy training and staff records.
An unannounced renewal inspection was conducted today, September 30, 2016 to determine the center's compliance with licensing standards. Upon arrival, the following ratios were observed:
Children ages 18months/2years: 1 staff/ 8 children (nap time ratio)
Children ages 3-4 years: 1 staff/ 9 children (nap time ratio)
School age children: 1 staff/ 8 children
Children were initially observed asleep on mats with the proper bedding. Once awake, children were observed using the restroom, eating snack, and participating in various activities. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All areas were inspected to include classrooms, main hallways and children's restrooms. The outdoor playground was inspected; however, due to previous rain an accurate measurement of the mulch could not be obtained.
The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, food menu, previous inspection, and various parent information. These postings are located on the bulletin board upon entering the center and in children's areas. The center's first aid kit and emergency supplies were inspected with additional supplies found. Annual health and fire inspections were reviewed and found to be updated. Eight children's and two staff records were inspected with the majority of the required information found captured, see attached notice. The following was also inspected: medication log, MAT certificates, fire/shelter-in-place drills, parent agreements, and other pertinent information.

Standard #: 22VAC40-185-60-A Description: Based on the inspection of children's records, the required information was not recorded.
During the inspection of eight children's records, child # 2 and #6 did not have the emergency contacts' full address listed. Licensing standards require both emergency contacts to have listed the full address and telephone number of each contact.
Plan of Correction: Per director: I will have the parents to complete the missing information and we will update the children's files.
Standard #: 22VAC40-185-550-D Description: Based on the inspection of evacuation drills, one drill was not conducted or recorded. Evidence: During the inspection of the monthly evacuation drills, it was discovered the evacuation drill for June 2016 was not conducted. Licensing standards require evacuation drills to be conducted monthly and recorded. Plan of Correction: Per director: I will make sure the drills are recorded each month.
