Comments:
An unannounced renewal study was conducted from 9:45 a.m.-1:15 p.m. with the center director. 62 children, ages ranging from 4 months through 5 years old, were in the care of 11 staff. Children were observed playing in the home living area, building with Duplos, listening to a Halloween readaloud, playing with toy dinosaurs and jungle animals, painting, and discussing community helpers with their teacher. Required information was posted. Lunch served today is whole grain, baked chicken nuggets, brown rice, mixed vegetables, applesauce, and milk. Both the fire and health inspections are current. The last emergency escape drill was conducted 9/26/16. The last shelter-in-place drill was conducted 6/16/16. The complete first aid kit, battery operated radios, and battery operated flashlights were present and accessible. There were no new medications to review. The playground and two buses were inspected and in compliance. Four children's records and four staff records were reviewed. Standard #: 22VAC40-185-150-B
Description: Based on review of four children's records, the center did not have immunization and physical examination documentation that was signed or stamped by a doctor, his designee, or local health department official. Evidence: Child A's record had documentation of immunizations and a physical examination that were printed by the pediatrician's office but not signed or stamped.

Plan of Correction: The director stated that she will notify the parent of this.

Standard #: 22VAC40-185-160-A
Description: Based on review of four staff records, the center did not have documentation of a tuberculosis test or screening for staff within 21 days of hire or within 12 months prior to hire. Evidence: Staff A?s (hired 9/8/16) record did not have documentation of a tuberculosis test or screening.

Plan of Correction: The director asked Staff A to retrieve the tb test documentation and return it after lunch.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not keep hazardous substances such as cleaning materials in a locked place that prevents access by children. Evidence: In the PreK-A classroom, a basket of cleaning materials that included Clorox toilet bowl cleaner, Lysol multipurpose cleaner spray, and Lysol power foam bathroom cleaner spray were in an unlocked cabinet under the children's bathroom sink.

Plan of Correction: The director removed the basket of cleaning materials and locked it away during the inspection.
