22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 9:45am-11:40am. Upon arrival there were 50 children in care with 9 staff present. A sample of 5 children and 3 staff records were reviewed in addition to 4 medications. The playground was not inspected due to area flooding earlier in the week. Children were observed in centers, playing musical chairs, listening to music and doing art. Infants were observed sleeping in cribs and on the floor with staff.
Standard #: 22VAC40-185-270-A Description: Based on observation the center failed to ensure areas and equipment of the center were maintained in a safe condition. Evidence: 1. In the 3-4 year-old room there was peeling paint along the door frame and in the toddler room there was peeling paint along one of the walls. 2. In the toddler room there was a vinyl couch that was torn in 2 places. The inside foam padding was exposed and accessible to the children in care. Plan of Correction: This work will be completed by 10/22/16. 2. The couch has been taped on 9/22/16 to cover the torn spots while waiting replacement. Teachers will complete a daily classroom checklist to ensure all areas and equipment are in safe condition. If there is any deficiencies teachers will report to the management team within 24 hours for a resolution. Our safety captain will be verifying checklists are complete. Management will also verify daily on a school manager checklist. DM will do walkthroughs on school visits.
Standard #: 22VAC40-185-430-I Description: Based on observation the center failed to ensure personal articles were individually assigned. Evidence: In the school age room there was a bottle of lotion in the supply cabinet that was not individually assigned. Plan of Correction: Staff will be retrained at the September staff meeting. Teachers will complete a daily classroom checklist to ensure all items are labeled. If there is any deficiencies teachers will report to the management team within 24 hours for a resolution. Our safety captain will be verifying checklists are complete. Management will also verify daily on a school manager checklist. DM will do walkthroughs on school visits.
