22VAC40-191 Background Checks (22VAC40-191)
An unannounced Renewal inspection was conducted during the program day on September 16, 2016. There were 14 children present with 2 staff members. Upon arrival, the children were observed in their classroom as they participated in varied activities. The LI also observed the children during the lunch meal and afternoon snack in the school cafeteria. Dismissal procedures were observed at the end of the program day. A sample of 4 children's records was reviewed. A sample of 2 complete staff records was reviewed at the administration office on 09/01/16. Required information for the Board Officers was reviewed also. A discussion was held with the Lead Teacher regarding the location of the posted License. The License is posted on a board on the far side of the classroom. A suggestion was given to post the License in the entrance area entering the classroom, conspicuous to the public. A discussion was also held regarding emergency supplies in the Center (activated charcoal preparation). The charcoal expiration date was 06/16, please dispose of properly. It is not required for the Center to restock the charcoal preparation. Clarification was given to staff regarding required ratios and direct supervision (Standard 350.A.) Standard #: 22VAC40-185-40-A Description: The Center failed to have the most recent inspection findings (02/04/16) posted. Evidence: Staff could not locate the last inspection report issued to the program. Plan of Correction: Staff will ensure the inspection report is posted in the Center with the License.
Standard #: 22VAC40-185-260-B Description: Based on reviewed documentation, the Center did not obtain a current report of environmental sanitation inspection form from the local health department to verify annual approval of meeting requirements for the water supply and sewage disposal. Evidence: The posted inspection report was dated 08/27/15. Plan of Correction: The Lead Teacher will follow up this week with the appropriate person to request an updated inspection report.
Standard #: 22VAC40-185-270-A Description: Based on direct observations, all inside areas were not maintained in a clean, safe and operable condition. Evidence: A ceiling tile in the program's classroom had a visible water stain. Plan of Correction: The stained tile was reported to maintenance dept. The Lead Teacher will follow-up.
Standard #: 22VAC40-185-550-C Description: Based on direct observations of the classroom, the Center failed to post emergency shelter-in-place procedures in a location conspicuous to staff and children. Evidence: The emergency shelter-in-place procedures were not posted. The LI verified this finding with the responsible staff member. Plan of Correction: The Lead Teacher will prepare written shelter-in-place procedures/maps to post in the classroom.
Standard #: 22VAC40-185-550-E Description: Based on staff report, the Center failed to maintain a record of the dates of the emergency practice drills for one year. Evidence: The LI reviewed documentation of the practiced drills for the current program year 2016 - 2017. Staff reported that documentation of the past drills was not available in the Center. Plan of Correction: The Lead Teacher will ensure complete documentation of the emergency drills is maintained for at least one year.
