Technical Assistance:
1. Reviewed fall zone and resilient surfacing requirements for playground equipment. 2. Reviewed hazardous substance storage requirements.

Comments:
Conducted an unannounced monitoring inspection 1:25-2:10pm. Observed 10 children + 2 direct-care staff in the preschool aftercare program. Ratios and supervision were in compliance. Staffing has remained consistent for a period of years. The younger children were napping, while the older children were having active play on the outside porch. The center was found to be clean and sufficiently supplied with developmentally appropriate toys and equipment. Hazards found in bathrooms and on outside playground. No medications reported to be on-site. Required documents were posted. Record keeping was in order. Questions about this inspection may be directed to: pamela.sneed@dss.virginia.gov

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observations made, it was determined that areas and equipment of the center were not maintained in a safe condition. Evidence: Hazards were found on the outdoor playground area, to include: (1) A nail head was sticking out of the wood, about 1/4 inch, on the top of the entry gate. (2) 2 wood timbers surrounding the large climber were broken and deteriorating with splintered wood. (3) The plastic platform of the cube climber was cracked and posed as a potential pinch point.

Plan of Correction: alert maintenance of repairs needed. To have nail fixed + new 2 x 4 wood pieces fitted. [sic] remove climber from playground By end of Oct. 2016

Standard #: 22VAC40-185-280-B
Description: Based on observations made, it was determined that hazardous substances were not kept in a locked place. Evidence: 1. 5 containers of household cleaning wipes were sitting on top of boxes/storage items just inside the doorway to the bathroom located across the hall from the 2's classroom. Evidence: The containers of wipes were within reach of children in care if they entered the bathroom. 2. A container of household wipes were sitting on a shelf above the diaper changing area in the bathroom located across the hall from the 3's classroom.

Plan of Correction: reminded staff that all cleaning products must be locked (not just out of reach) all cleaning products behind locked cabinets

Standard #: 22VAC40-185-330-B
Description: Based on observations made, it was determined that climbing equipment did not have a minimum of 6' fall zone surrounding it. Evidence: (1) The small climber/slide and the cube climber had between approximately 3'-4' of fall zone. (2) The small slide was against the wooden fence and had no fall zone on one side.

Plan of Correction: Move equiptment to ensure proper fall zone. [sic]

Standard #: 22VAC40-185-560-F-4
Description: Based on observations made, it was determined that a snack menu was not posted for 1 of 2 classrooms. Evidence: There was no menu for the 2's classroom available for review.

Plan of Correction: Snack menu posted in each room.
