An unannounced monitoring inspection was conducted on September 28, 2016. Upon arrival to the facility, there were two staff present with13 children. Children were playing with various table top toys and activities and transitioning to school. Three children's records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, nutrition, injury report documentation, children's records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, emergency supplies and required postings. There are currently no medications being administered at this time. The outdoor play area was not inspected due to inclement weather. The provider's response for the "plan of correction" was not received as of 10/09/16 and will not appear on this Violation Notice.
Standard #: 22VAC40-185-60-A Description: Based on record review in one of three children's records reviewed, the licensee did not ensure that each child's record was documented with all required information. Evidence: During the inspection conducted on September 28, 2016, the record for Child #2 did not include documentation of employment information for one parent and the name of any previous child day care and schools attended by the child. Standard #: 22VAC40-185-550-D Description: Based on observation and inspection of the facility, the licensee did not ensure that a monthly practice evacuation drill was conducted.
