Comments:
An unannounced monitoring inspection was conducted on October 5, 2016. The director was not present and one of the staff assisted with the inspection. There were 2 toddlers and 13 preschool age children (2 to 3 years) in care with 2 staff. All of the children were sleeping on mats in the main activity room under the sight and sound supervision of the staff. There were no medications being administered at the center. Four children's files and 2 staff files were reviewed. Evacuation drills and shelter in place drills had been conducted. Inspection began: 1:00 Inspection ended: 2:20

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a review of 2 staff records, the center did not obtain the results of TB test/screening at least 2 years from the date of the initial screening. Evidence: The TB test/screening on file for Staff # 1 was dated 7/30/14.

Plan of Correction: Staff # 1 did not return to work until TB results were done.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the outside area of the center was not maintained in a safe condition. Evidence: The areas of the playground and equipment were observed to be in disrepair and unsafe. 1. Five rusty screws at the latch on the gate were accessible to children. 2. Pickets at the fence were pulled away from boards and rusty nails were exposed and accessible to children. 3. A board on the privacy fence was splintered and broken. 4. A riding toy and a small slide were split and and cracked with openings that could pinch or snag skin. 5. Grass was several inches tall in the play area that did not have mulch.

Plan of Correction: Exposed nails and screws hammered in until: (10/5/16) 1. New panels on fence installed - 10/15/16 2. New panels on fence installed - 10/15/16 3. New panels on fence installed - 10/15/16 4. Toy was disposed - 10/5/16 5. Grass was cut - 10/6/16

Standard #: 22VAC40-185-330-B
Description: Based on observation, the center did not maintain the required resilient surfacing at fall zones for slides. Evidence: The fall zone at the end of the curved blue slide measured less than 1 inch of depth. The fall zone at the end of the straight blue slide measured 1 to 2 inches of depth.

Plan of Correction: Mulch was ordered for playground and will be delivered.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not have a diaper disposal that was not hand generated at a diapering area. Evidence: The diaper disposal in the infant room was hand generated.

Plan of Correction: Hand generated diaper disposal was replaced by foot generated.
